Information for new patients

Welcome to Richmond Road Dental, and thank you for placing your trust in us. We hope that you’ll enjoy being part of our dental family.

To prepare for your first appointment, please download and complete the following forms:

  • A medical history form.
    • Please bring this form with you to your appointment. 
    • If you’d prefer to fill out this form in our practice, please arrive ten minutes before your appointment.
    • If you want to know why we ask so many questions about your medical history, please read this factsheet from the Australian Dental Association.
  • New patient survey: Please answer some questions about your first appointment. You can answer anonymously; we don’t collect your contact information in this form.

Parking

The practice is situated on the corner of Richmond and Molloy Roads. Convenient off-street parking is available behind the building, accessed from Molloy Road. Street parking is also available on Molloy Road. 

Arriving by public transport

The practice has easy access to a major bus route and is close to the Morningside train station.

Children

Children of all ages are welcome in our practice. There is a dedicated space for children to play, where the toys are sanitised twice daily. Your child is welcome to browse and enjoy our fantastic assortment of children’s books, including many about visiting the dentist. If your child enjoys colouring or puzzles, they might enjoy visiting our colouring table. You are, of course, welcome to bring your own toys and books along.

While our team are happy to hold your baby or temporarily distract a child while you visit the loo or chat with your dentist, we cannot supervise young children.

Accessibility and inclusion

Our practice and bathroom are wheelchair accessible.

All breastfeeding parents are welcome in our practice.
